Instructions for Filing Description Section on Transmittal Form
The Filing Description section on the NAIC transmittal serves as the cover
letter for the filing. Include a complete explanation of the marketing program,
include specific items that differ from the company's general portfolio.
Explain how new forms differ from similar forms, any changes to revised forms
or the reason to file a new form for the same product. Describe any innovative
or unique features of each form. The certificate of compliance form must be
signed by an authorized person and all relevant standards completed. Failure to
attach all the requirements identified on our Web site as submission
requirements will result in disapproval.
Filing fees are not assessed in Oregon.
For paper filings, submit two sets of the entire filing with actuarial data (if applicable) and two, self-addressed, stamped envelopes, one large enough to return the final approved filing.
Communication will be corresponded by e-mail, fax, or phone when possible. Filings that contain errors will be rejected without review as incomplete filings. We will let you know the reason for this
action and will accept corrections to these filings if received within 30 days. If we contact you about compliance-related concerns or corrections that need to be made to the accepted filings, we
must receive your complete response within 10 calendar days. If this is a resubmission of an incomplete filing, it must be submitted as a new filing and reference the previous submission verifying
all the changes are made as previously requested.
